Here at Wokingham, we have two inpatient wards, caring for adults with a wide variety sub-acute medical and post-surgical conditions. Our patient cohort is referred from both acute and community setting. Without immediate access to acute investigations, we use multiple 'Point of Care Testing' systems to support the care of our patients. Our diverse range of staff are skilled inappropriate managementof patients, supporting them toremainin the community setting, whilst also escalating asappropriate toensure their safety. We are looking for an experienced nurse to join our growing team to provide this continued level of care to our patients. • Work closely as part of theMulti-DisciplinaryTeam for the care of patients requiring medical and rehabilitation support,predominantly postan acute hospital stay.
• Responsible for the assessment of care needs,developmentand implementation ofpatient centered care. Promoting independence and safe discharge home.
• Lead in ourNurse led unit, especially during out of hours shifts.
• Shiftwork andshiftmanagement,working with junior members of staff in all aspects of patient care,participatingin training and support.
