Full Job Description

An exciting opportunity has arisen to work with and support our skin cancer specialist nurses as a pathway navigator.
This role will support early diagnostics and treatment pathways for patients with skin cancer.
The successful applicant support the CNS's and wider multi disciplinary team in delivering high standard of support for our patients. They will be expected to liaise with GP's, co-ordinate diagnostic tests and communication with local and regional Cancer MDT's
This is predominantly an administrative role however we welcome Registered Nurse Associates to apply with potential to expand and develop the role in the future, To communicate regularly with patients to ensure that they are aware of upcoming dermatology-plastic combined appointments, diagnostic tests/investigations and operations dates and offer practical support with the arrangements and provide consistent planned follow-up to reinforce and further promote this information.
In conjunction with the Clinical Nurse Specialists (CNS) provide coordination of high-quality patient care through ongoing telephonic interactions with patients and proactive identification of needs such as WHO performance status, hospitalisation within this trust or other trusts, pregnancy, language barriers and arranging language interpreter during clinics to improve patient experience.
To ensure that the provisions of appropriate services at the right time and in the right place are available for patients and to have the knowledge to signpost patients as required., The post holder will improve patient experience by providing support and non-clinical information throughout the entire cancer pathway and reduce patient anxiety by providing reassurance and information on interventions/tests, surgery appointments and ensuring patients arrive for interventions fully prepared.
Refer complex clinical decisions to the skin cancer Clinical Nurse Specialists for assessment and review.
Assist Clinical Nurse Specialists to develop education, information resources and to collate the information for patients and other professionals regarding all aspects of cancer care.
