Full Job Description

The West Devon Core Team are looking for an experienced, skilled and enthusiastic worker to join our established community team.

This is an exciting time to join community mental health services as we progress with the CMHF-community mental health framework-. The post holder will be expected to undertake a range of duties including assessment, care planning and psycho-social interventions. It is envisaged that part of this role will be based in the Ivybridge location fo our patch developing links with local GPs and the local PCN

The team is established with strong clinical and managerial leadership. The right applicant will be offered suitable training opportunities and career development in clinical or managerial areas.

Main duties of the job

Triage and assessment of referrals from GP and PCNs

Care planning and intervention with individuals requiring secondary care

Attending MDT and multi-agency meetings as required

Undertaking relevant training and ensuring professional qualifications/registrations are maintained

Supervising staff as required

Supporting the teams group programme
