Full Job Description

We are looking for motivated, experienced professionals embarking on a new career, or returning to work, to join us working in partnership with multi-agency teams, ready to embrace change and new ways of working. If you are a Social Worker with experience in Adult Social Care then we would love to hear from you.

As a Social Care Discharge Co-ordinator, you will work with the Rapid Transfer of Care team (RTS) to manage the discharge pathways for a population of clients moving through the acute hospital setting, working closely alongside other agencies and professionals in order to achieve this.

You will be responsible for triaging and allocating referrals to appropriate workers. You will supervise and provide complex case management advice to a team of Case Officers and Social Workers. You will support the team to deliver a high quality of practice and innovative person centred outcomes for the residents of East Kent. You will also be responsible for accurate data collection and escalating any issues affecting functionality of the team to the Team Manager. The RTS team operates a discharge to assess model of care which involves patients having their assessments outside of the acute hospital environment and you will be supporting clients as they move through and beyond this pathway.

The service is increasingly accessible to the public outside of traditional office hours, as we move towards an 8 am to 8 pm, 7 days a week service. OPPD staff and managers are increasingly working on a rota basis to ensure service needs are met. - Starting salary £37,374 to £43,592, This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements.
