Full Job Description

The Team has an opportunity for a Social Care Officer to join their dynamic and dedicated team which provides mental health social care provision in Hartlepool to all age adults (18+) with mental health needs., You will be required to complete annual reviews and social care assessments of health and social care provision regarding commissioned services and direct payments. You will also need good administrative skills, you will have proven experience of developing, maintaining and inputting information on electronic based systems in order to provide timely management information. You will also be able to demonstrate sound analytical skills and effective decision-making within the appropriate timescales and will demonstrate a person-centred approach and the ability to deal in a tactful and sensitive manner with people who access our services.

The successful applicant will receive regular supervision and an annual appraisal, they will be well supported and have opportunities in respect of their career development with the potential to experience a variety of roles within adult social care.

Hartlepool Borough Council is committed to safeguarding and promoting the welfare of children and vulnerable adults. The successful applicant will be required to undertake a DBS check via the Disclosure and Barring Service (previously known as the CRB).

You will have recent relevant experience of working in a Social Care setting and of dealing with enquiries from members of the public or other agencies. A sound knowledge base and understanding of roles and responsibilities within Social Care is essential, ideally including knowledge of current developments and service pathways.

This new approach coupled with significant investment into the mental health system has produced closer working relationships with Primary care, the Voluntary and Community sector and our colleagues within the Mental Health Trust.
