Full Job Description

We have an exciting opportunity for a social care practitioner to join the Discharge Assessment and Review Team.
We are a newly developed team geared to review people's care and support that has been put in place after being discharged from hospital.
The team is a countywide team. However, for this particular post we are looking for people willing to work in the Chesterfield, Bolsover and North East Derbyshire area. The post involves travel as it requires regular attendance to provisions to complete assessments & reviews.
Experience of working in Adult Care, is beneficial but not essential. You will need to have transferrable skills to enable you to facilitate safe, sustainable transfers of care for people leaving provisions utilising a Home First approach.
The post is for a 5 in 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis. Prior notice will be given in advance if this changes.
You will need to be able to manage change and have the ability to work effectively and build positive relationships with health partners and other agencies. You will need to be committed to working in a person centred and strengths-based way with clients and their families to maintain and maximise their independence, whilst observing the tight timescales under the Care Act and current guidance.

Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
