Full Job Description
An opportunity has arisen for a Social Care Practitioner to join us in Amber Valley East Adult Social Work Team. We are a busy, supportive and dynamic Adult Social Work Team, supporting adults over 18 and their families and carers.
You will be based within Amber Valley East Adult Social Work Team. There is an expectation you will be flexible worker, office based in Alfreton for part of the week and also working from home or other bases around the County.
Training and induction will be provided alongside daily team meetings, and regular management and peer supervision. You will need to have experience/ understanding of assessing and reviewing clients needs in the community or residential settings.
Working in partnership with people and their carers, you will create person-centred and strengths- based assessments and support plans and commission services on their behalf to enable individuals to meet their outcomes, promoting a home-first culture.
You will need the ability to travel and ideally have access to your own transport for which a mileage allowance is payable. We have a supportive and friendly team ethos. We promote ongoing learning and development with a strong focus on the wellbeing of the team.
The work can be fast-paced, challenging and stressful at times but varied and extremely rewarding in a nurturing and protective environment. We are seeking a motivated individual with a zest and passion to promote best practice and join us in improving the client journey for Amber Valley residents.
