Full Job Description

An opportunity has arisen for a Full time Social Care Practitioner to join South Derbyshire North Adult Social Work Team . We are a supportive and person centred team supporting young adults in transition and adults over 18 years of age.

The successful candidate will be based in South Derbyshire with hybrid working . Training and support will be provided which includes regular supervision , peer support , team and area meetings alongside access to Derbyshire County Council training .
You will work in partnership with clients , their families and carers to undertake Care Act (2014) assessments , create support plans and regular reviews . This will be through person centred and strength based approaches to commission support and signposting promoting the home first culture . The team also supports residents for whom their are safeguarding concerns and works with The Mental Capacity Act (2005).
You will also work with other stake holders for example care homes , housing departments , NHS and many others to support clients through a multi disciplinary approach .
There is an expectation of travel through use of your own transport for which a mileage allowance is claimable .
