Social Care Practitioner (LAP)

The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital.,

• Assessments and interventions for residents with physical disabilities, not excluding cognitive decline
• Working with other professionals to support identified outcomes
• Holding and managing a case load
