Social Care Practitioners are based in teams across Adult Social Care with qualified Social Workers and Managers, and work with the people of South Tyneside who have care and support needs. The role involves building excellent relationships with people, their families, and carers to understand what is already working well and what really matters to them. Using creative thinking, knowledge of the community, our partnerships and services, Social Care Practitioners help people, regardless of their circumstances, achieve their goals for a safe, happy, healthy life. Main Responsibilities:
- Building relationships with adults, families, and support networks to identify strengths and design and agree ways to achieve the person's goals using our Let's Talk Together Approach.
- Undertaking visits to meet with adults face to face at various locations within borough, and occasionally beyond.
- Being curious and asking questions to identify potential risks which need to be considered and addressed.
- Being committed to keeping adults safe where risks are identified in line with Making Safeguarding Personal.
- Maintaining accurate records that represents the person's wishes and goals and demonstrates any decision-making rationale.
- Participate in meetings with other professionals, advocate for the person and strive for the best outcome.

You will think 'outside of the box' and must have the drive and ambition to make a positive contribution to your team and the service. You might be a school leaver or recent graduate looking for the next step. Or perhaps you work in the care sector and are looking for an opportunity to progress.
