Full Job Description

The STAR service is Sandwell MBC inhouse short term assessment and reablement service working with people in their own homes to promote independence when people leave hospital or to prevent a hospital admission occurring. The service plays a pivotal role in our Discharge to Assess (D2A) pathways, working closely with hospital and community-based partners. We are currently looking to recruit a new Team Manager to manage and further lead the development of this fast-paced service as it continues to provide reablement focused services to our customers. The service is a regulated service with CQC so the successful candidate will be required to become a Registered Manager in order to deliver this function. The post has responsibility for a team, which is made of a Deputy Manager, Social Care Co-Ordinators, Team Leaders, Trusted Assessors, Schedulers. Senior Home Support Assistants and a group of Home Support Assistants who deliver the reablement support. The post holder will also work alongside other managers from both Health and Social Care as part of the D2A system. The ideal candidate needs to demonstrate good managerial experience which delivers high quality outcomes to people as well as possessing leadership and change management skills to evolve the service as demand in the system changes.

You are required to hold a professional qualification at degree level and/or NVQ 4 or equivalent. Knowledge of equipment provision, clinical assessment.

The post holders contract is for 37 hrs per week but may need to work those hours flexibly to manage the demands of the service which operates 7 days per week from 07:00 to 22:30 We offer: -
