Full Job Description

An interesting and exciting opportunity has risen for Band 6 Social Care and Complex Case Lead. We are looking for an enthusiastic, motivated and proactive registered nurse to join our healthcare team at HMP Holme House. The team includes a wide range of experienced, skilled individuals including nurses, clinical support workers, pharmacy technicians, pharmacist, ANP, physiotherapist, administration support workers and more.
You will become part of a supportive multi-disciplinary team, using your skills, experience and leadership to help define and deliver care to those with often complex needs. As well as providing excellent care to service users, you will also assist other senior managers in supporting and developing staff within the team.

There is also free parking., The post holder will provide a comprehensive primary care physical/mental health service as part of the Multidisciplinary Team to prisoners with health needs and will contribute to and deliver a comprehensive primary care health service. As team Leader you will operate as part of a multi-disciplinary team, assisting and supporting others in undertaking physical and/or health assessments, providing a range of health treatments and interventions in order to meet prisoner healthcare needs and contribute to the wider continuous development of prison healthcare services. You will be responsible for the day-to-day line management of the Primary Care Team and will ensure that the delegation of work to the team takes account of the role, skills and competence of staff and is appropriate to their scope of practice. This will include being able to demonstrate a vast range of knowledge, skills and understanding of health issues and treatment options to provide clinical guidance to all
staff on matters relating to physical health and wellbeing.
The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care demonstrated through the advanced competency framework.
You will also support other senior managers in ensuring a high level of performance within the team is maintained, by participating in audit, Health and Justice Performance indicators and NHS data collection activity., We're looking for passionate individuals to join our diverse and innovative team.
