Full Job Description

Social Prescribers also work collaboratively with community groups and service providers so that these remain accessible and sustainable. Social Prescribing helps to strengthen personal and community resilience and reduce health inequalities by addressing the wider determinants of health such as debt, housing and physical inactivity and promotes active involvement within local communities.
