Full Job Description
This role is working for SYMPHONIE PCN (Primary Care Network) across a group of 5 GP surgeries, based mainly in the West Hull area. The main base is located at the Wilberforce Health Centre with some clinics held at the surgeries. You will be a part of team working to support patients in our PCN.
Social prescribing empowers people to take control of their health and wellbeing working alongside existing social prescribers and health and wellbeing coaches.
Link Workers support existing voluntary, community and social enterprise groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.
The service provides a one-stop connector service for a wide range of community related support with an emphasis on loneliness and isolation, and on the determinants of health, particularly low income, employment, learning, support, housing, debt, financial management and domestic abuse. Link Workers play a pivotal role by developing trusting relationships and providing personalised care and support plans., The candidate will work as part of the team to support referrals from surgeries on loneliness and isolation, and on the determinants of health, particularly low income, employment, learning, support, housing, debt, financial management and domestic abuse amongst many things. You will liaise with patients, community providers and the GP surgeries.
Link workers play a pivotal role by developing trusting relationship's and providing patient personalised care and support plans., The Social Prescribing team works closely with Health Well Being coaches as well as with all PCN team members and member practices.
The Social Prescribing team currently consists of a 2 social prescribers, with one a team leader, and this will be an addition to the current team.
