Full Job Description

o To manage a complex caseload of service users as part of a hospital avoidance and discharge pathway and to co-ordinate care in the context of relevant health and social care legislation and policy frameworks including but not limited toThe Care Act 2014 Mental Health Act 1983/2007 Mental Capacity Act 2005 Interagency Adult Safeguarding policies and procedures CHC Framework/Guidance

o To contribute to the assessment and care management process within a multi- disciplinary team

o To make service users aware of financial contributions, to provide information enabling service users to make an informed choice.

o MDT working in both community and hospital settings (The Royal Liverpool Hospital Frailty Unit, Longmoor House Aintree Hospital, Liverpool Innovation Park)., Complete assessment visits - assess patients/service users needs and identify support required to support their discharge from hospital.

To follow relevant legislation and law - Care Act 2014, Human Rights Act 1998 and Mental Capacity Act 2005.

To complete Care Act assessments, support plans, mental capacity assessments and best interest decisions and Transfer of Care documents.

To access/use relevant systems such as liquid logic, EMIS, PENS etc.

To follow social work code of conduct and follow all in house procedures, and confidentiality, GDPR etc.
