Full Job Description
We're a team of professionals that work together to support local adults with their health and social care needs, helping them to maximise their independence and live healthy lives. Based in the community in GP surgeries, you'll work in partnership with occupational therapists, physiotherapists, social prescribers, GPs, nurses and mental health practitioners to empower people to live healthy lives in their own homes. You'll already have good communication and listening skills, and you'll care about helping people have a greater voice, choice and more control on how their needs are met. This is an ideal role for someone who likes to apply creativity to help people make best use of their own assets, along with community services. This role is part of our wider adult social care service and we take great pride in the development of our workforce. - Carrying out strength-based conversations as outlined under the Care Act 2014 for adults and their carers.
- Supporting adults and carers to meet their eligible outcomes in the most resourceful way.
- Demonstrating an ability to use the council's email, social care data base and electronic recording system and any other systems deemed essential to the role.
- Being aware of quality standards and performance measures and ensure they are adhered to.
- Providing Practical Support where needed to individuals.
Diploma 3 or above in Health and Social Care/Supporting People or equivalent. - Experience working in a social/health care setting.
- Driving licence and has access to a car to get to various locations across Milton Keynes.
- Good communication skills with individuals and their diverse needs.
- Ability to work as part of a team including working in partnership with other professionals and agencies.
- An awareness of adult abuse and Mental Capacity.
- Able to work autonomously but recognise when support is needed.
- Good written and recording skills.
- For this role, the candidate is required to have a ull UK driving Licence, along with access to a Vehicle. Evidence of the driving licence and vehicle insurance with business Insurance will be required from the candidate should they be offered this position. Without this documentation the candidate will not be appointable.
- Should you be offered the role, as part of the pre-employment checks an Enhanced DBS will be actioned for you (unless you are on the DBS Update Service).
