Full Job Description
The post holder will have the opportunity to work jointly with other members of the IST within a shared caseload of people who require intensive support. This is a unique opportunity for an experienced Social Worker to be able to work in a focused way with people with a learning disability and their carer's.The post holder will be expected to provide specialist social care input to contribute to the assessment and support planning for service users with a learning disability who experience complex behaviours and/or mental health needs. This will include working collaboratively with other disciplines and agencies in order to provide the highest quality service to service users. The Intensive Support Team consists of nurses, psychologists, occupational therapists, Speech and Language Therapists and assistant practitioners and works closely with the local Community Learning Disability Team, and Dynamic Support Register Service.
The IST provides services to adults with Learning Disabilities and behaviour that challenges. The team is based in Stafford and covers the South Staffordshire area. The post holder will engage with service-users in a variety of settings.The successful applicant will be joining a team with an outstanding value base, and high expectations of supporting good quality of life outcomes for the people they serve.Previous experience of working with people with learning disabilities is essential, preferably as part of a community team.
