Covering North Norfolk and Broadland our service provides an integrated Health and Social Care approach to assessment and support in the community. You will need to have a positive attitude and enjoy finding creative and innovative solutions. You will need to have excellent communication skills, record keeping and keyboard skills. You should have a commitment to excellent customer service and be confident in strengthening Social Care Practice. Norfolk County Council and Norfolk Community Health and Care Trust deliver together a single service for adults with a learning disability. We are an integrated Health and Social care team providing specialist learning disability support to adults in the community. You would be joining an inspiring and dedicated team of Social Care practitioners who are focussed on supporting adults with a Learning Disability to live independent and fulfilling lives.
