Full Job Description

Carrying a caseload of older adults with a mental disorder and/or dementia who have been admitted to acute or psychiatric hospital; Planning and delivery of outcome based assessments, reassessments and support plans which are effective in promoting independence and meeting individuals outcomes and aspirations and in line with the strengths based approach. Facilitating safe and timely discharges in line with the Care Act 2014; Use relationship building, professional curiosity and analytical skills to inform assessment, decision making and intervention. Complete support planning with the person, to ensure that outcomes identified at assessment are met in line with the persons priorities, within budgetary constraints and through the provision of support, equipment, adaptations to promote independence and prevent, reduce and delay any increase in need. Take a professional lead and oversee cases of individuals who lack capacity to agree to or plan their care, undertake proportionate
assessment in accordance with current Adult Social Care legislation including MCA and best interest assessments. To act as the lead professional to services users and their carers and families to enable them to retain, as far as possible, choice and control of their lives. Support in safeguarding processes, to assess and manage risk, knowing how to intervene proportionately and ensuring people are protected from harm, while protecting their human rights and taking a Making Safeguarding Personal approach.
