Full Job Description

Are you an experiences Social Worker with who is looking to enhance your knowledge and skill in mental health working?, The successful candidate will:

  • hold a strong belief in recovery for people with severe and enduring mental health problems
  • hold a caseload and undertake the role of care coordinator for those with varied complex mental health and social care needs
  • have experience of working in a community based setting
  • will provide direct support and advice to Service Users (Adults of all ages) and Carers, within the recovery framework, embracing the individual right to choice and control
  • ensure that appropriate health and social care services are commissioned as required
  • will also assess, identify and evaluate Service User's needs through comprehensive assessments, reviews, care planning and CPA in conjunction with other professionals.

  • This role attracts a Golden Hello of £3,500, new employees only.
