Full Job Description

An opportunity has arisen in The Transformation and Partnership Team, who are looking to recruit a qualified social worker who is suitably experienced, and passionate to develop their skills and knowledge with young people preparing for adulthood in a strength-based outcome focused way as part of an adult social care team, within the wider county-wide Transformation team.
This is a part-time post (22.2 hours) for a fixed term until 26/02/2026 or internal secondment.
Transition to Adulthood looks at using innovative forward-thinking approaches including assessing and reviewing young people to achieve person centred, outcome focused and strength-based approaches that will meet their assessed eligible social care needs in creative ways and by promoting independence.
The successful candidates will be experienced working in Adult Social Care and have knowledge of the Care Act 2014 and The Health and Care Act 2022.
Experience of working with young people who may require support in their journey into adulthood is highly desirable, as is having a good understanding of the 'six themes of making it real' embedded within Think Local Act Personal (TLAP). We are looking for individuals who are passionate about creating the best opportunities for young people going through transition on their journey to adulthood to have great lives that include jobs, friends, homes, and communities they love. Good communication skills are essential.
Core tasks will include completing strengths-based timely assessments, reviews, support planning, and working in collaboration with colleagues including Children's Services, Adult Social Care Personalisation and Prevention teams, SEND, Health and education to achieve positive and more independent outcomes for individuals.
An ability to travel is essential due to this role being county wide.

SW Grade 9-11 - An initial one off lump sum 'Welcome payment' of £1500 (pro-rata) is attached to this post, payable in your first salary. If you are a qualified AMHP and practice as an AMHP in your post, you will instead receive a 'Welcome payment' of £2500 (pro-rata), payable in your first salary. If you voluntarily leave your employment with Derbyshire County Council within 12 months, you are required to repay some or all of the payment (Terms and conditions apply).
