Full Job Description

We are seeking an experienced Social Worker to join LOROS Hospice. In this role, you will provide expert guidance on safeguarding. You will manage the Social Work Liaison Officer, act as Deputy Designated Safeguarding Lead, and deliver person-centred care to patients and their families. Responsibilities include conducting holistic assessments, supporting multidisciplinary teams, and providing welfare advice.   • Conduct social care assessments for patients with complex needs, using holistic social work interventions.
  • Provide flexible support services to patients, families, and carers.
  • Support safeguarding processes, liaising with statutory services, and assist in policy development.
  • Line manage the Social Work Liaison Officer.
  • Act as Deputy Designated Safeguarding Lead, assisting with safeguarding matters.
  • Offer practical support, signposting patients and families to relevant services.
  • Provide welfare rights advice and guidance to patients and families.
  • Assess carer needs and make referrals to external services as necessary.
  • Advise staff on safeguarding concerns, manage referrals, and update records accordingly.
  • Deliver safeguarding training, covering key areas like MCA, DoLS, and PREVENT.
  • Promote good safeguarding practices, supporting policy, legislation, and integration of MCA and DoLS.
  • Manage administration tasks, ensuring accurate and timely records, and support the Social Work Assistant with theirs.
  • Collaborate in multi-disciplinary teams, attending and contributing to meetings and coordinating complex discharges and case conferences.
  • Work with internal teams to support safe hospice discharges.
  • Liaise with external agencies, such as Adult and Children Social Services, local clinical groups, and acute trusts.
  • Support the team in discussing serious illness and death with children when needed.
