Full Job Description

We have an opportunity for a qualified Social Worker with experience at Band 6 level to join our friendly, supportive team at the highly specialist and well sought-after Blackheath Brain Injury Rehabilitation Unit in London. Located in trendy South East London, 10 minutes-walk away from the pretty cafes and boutiques of Greenwich, the unit has a 19-bed Level 1 Highly Specialist Cognitive-Behavioural ward and a Level 2 Neurorehabilitation ward with 18 beds. Both wards provide specialist treatment to patients with physical and cognitive impairments resulting from an acquired / traumatic brain injury. The successful candidate will work as part of a wider, experienced and passionate multidisciplinary team (neurorehabilitation medicine, neuropsychiatry, neuropsychology, nursing, physiotherapy, speech and language therapy, occupational therapy, music therapy, social activities coordinators) to support each patient to achieve their planned outcomes. Responsibilities:

  • Manage the service provision for a defined caseload of patients / service users
  • Ensure the service meet the needs of the legal framework DoLS
  • Act as the lead in the assessment and implementation of care through to point of discharge
  • Develop and deliver evidence-based packages of care including specific psychosocial interventions where appropriate.
  • Work in partnership with service users, carers, other team members and other statutory and non-statutory agencies.
  • Liaise closely with other agencies at the point of pre discharge to ensure a smooth transition.
  • Undertake care planning
  • Maintain contemporaneous health and social care records and comply with relevant ACG polices
  • Keep people safe from harm and protect their human rights
