Full Job Description

The Newton Abbot Community Health and Social Care Team are looking for an enthusiastic, positive and person-centred social worker to join our disability focus team, supporting adults under the age of 65 with physical and learning disabilities, mental health difficulties and acquired brain injuries. The Adult Social Care Team are integrated and co-located with health colleagues all under the management of a health and social care Community Services Manager. We work closely with our primary and secondary health colleagues, providers and other social care teams to ensure we are providing a holistic approach to supporting our local population. We seek to build excellent links and relationships with those around us, working positively and professionally at all times. We are a busy team covering a large area of South Devon with a range of providers and community opportunities. Our duties include social care assessments and care and support planning, reviews, mental capacity assessments and S42 safeguarding. We work in a strengths based way to support individuals and their families to resolve what is going on for them in a way that works well for them. Based in our central Newton Abbot offices, you will have flexibility to work from home for part of the week as part of our hybrid working policy. You will ideally have experience of social work with younger adults with disabilities and will have strong communication, organisational, and interpersonal skills. You will have the ability to work independently and as part of a multidisciplinary team. You will have a commitment to promoting equality, diversity, and inclusion, and to supporting adults with disabilities to achieve their potential. You will join a progressive, supportive team and will receive regular supervision. Social Worker Career Pathway We have implemented the first phase of a new Social Worker Career Pathway within our Adults Services, with our vacancies available at the Social Worker (Grade F) or Experienced Social Worker (Grade G) levels. Our Social Worker and Experienced Social Worker roles align with the national Professional Capability Framework (PCF) for Social Workers, which provides a robust structure for all local authorities to use as a way of standardising professional capabilities and provides a clear career pathway. This is a fantastic opportunity to provide you with an achievable, clear pathway for your career; to enable you as a practitioner to reflect and identify your own practice and development on the PCF pathway. If successful in being shortlisted for interview, your experience, skills and knowledge, will be assessed against the PCF domains and if found appointable, this will determine your appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G). With on-going learning and support from your line manager, those appointed as a Social Worker will be able to evidence incremental learning and submit an application in due course for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
