Full Job Description

North East Derbyshire South Adult Social Work Team are looking to recruit a qualified and experienced social worker based at Clay Cross, Chesterfield.
This is a busy team based in the community and consists of Social Workers and Social Care Practitioners who work closely with partner agencies.
The successful applicant will be able to demonstrate the necessary skills, knowledge, and experience to undertake this role to provide a safe, accountable, cost effective and efficient service to our clients which helps them to achieve their outcomes. Key responsibilities will include carrying out Care Act 2014 assessments and reviews for individuals with complex needs as well as arranging the provision of services to meet eligible needs. It is expected that you will be committed to working in a person centred and strengths-based way with clients and their families to maintain and maximise their independence.
An understanding and experience of safeguarding is also essential as well as experience of undertaking capacity assessments in accordance with the Mental Capacity Act 2005.
There is an expectation that you will act as duty worker on rota basis which will involve triaging new referrals to the team and supporting clients and carers who require a more urgent response.

You will need to be able to manage change and have the ability to work effectively and build positive relationships with health partners and other agencies.

SW Grade 9-11 - An initial one off lump sum 'Welcome payment' of £1500 (pro-rata) is attached to this post, payable in your first salary. If you are a qualified AMHP and practice as an AMHP in your post, you will instead receive a 'Welcome payment' of £2500 (pro-rata), payable in your first salary. If you voluntarily leave your employment with Derbyshire County Council within 12 months, you are required to repay some or all of the payment (Terms and conditions apply).
