Carrying out Mental Capacity Assessments and Best Interest decisions, Multi-agency meetings, working with partner organisations and colleagues, the person and families to provide effective outcomes that meet the assessed needs of residents of Shropshire.

Completing CHC checklists and participating in Decision Support tool meetings.
