Full Job Description

The post holder will work within the Adult Mental Health Team providing social care expertise and care coordination.The post holder will ensure that all service users with the AMHT have full assessment of their Care and Support needs under the Care Act 2014 and that support planning takes place to meet these needs.The post holder will identify and address any barriers to promoting a social model of mental disorder, in partnership with Service User, their family and support network and the Multi-Disciplinary AMHT.The post holder will provide expertise for inpatient staff with regards to Adult and Children's Safeguarding responsibilities.The post holder will provide expertise with regards to application of the Mental Capacity Act and Mental Health Act.The post holder will demonstrate the skills and aptitude to maintain their Social Work professional identity when working within a Multi-Disciplinary team, 1.To work independently as part of a multi-disciplinary team to: assess, plan and implement patient specific programmes of care/interventions within the Care Programme Approach, contributing to the continuous holistic assessment of patients.2. To participate in and contribute to the multi-disciplinary team, articulating the social care perspective and ensuring that meeting the social care needs of service users retains a high priority and profile within the functioning of the team.3. To undertake Care Act assessments some of which will be for patients presenting with highly complex needs using your profession specific knowledge and skills.4. Ensure that there are no delays to service users receiving appropriate care and support by completing the relevant process to access care funding and implementation of support plans.5. To function effectively across a variety of environments (e.g. AMHT, ward, department, service users homes etc.) involving changing and demanding conditions
