PO3 - £43,131 to £46,016 per annum inc Market Factor payment A new exciting opportunity has arisen within the Adults with Learning Disabilities and Autism Team for a social worker to join us. We work with adults from the age of 18 who are diagnosed with a Learning Disability and/or Autism across Telford and Wrekin. Whilst we are an adult social care team, we also support all young people from the age of 14 who are preparing for adulthood and transitioning into adult services.
The ALD Team are passionate about supporting people to live fulfilling lives as valued members of their community. A key area of focus in enabling people to live independently and maximising their independence, as well as supporting carers to maintain their role. The Team also supports young people to prepare for adulthood and the multiagency working is vital in ensuring people have the right support that meets their individual needs and desired outcomes. "It is a small but supportive and approachable team, in a lovely location in Shropshire without compromising the convenience of multiple nearby cities" You will be expected to work in a person-centred way to enable adults who are Autistic and/or diagnosed with a Learning Disability, to reach their full potential, maximise their social inclusion and achieve positive outcomes. We use strengths and asset-based approaches in our practice; how individuals can be supported by the strengths they have in themselves, their network of support from family, friends and their local communities. Where care and support is required, we work to ensure that this reflects the individual's assessed needs and strengths while promoting their independence, choice and control. Our Social Workers will work with the individual, their carers and families, to build a Support Plan, with a broad range of interventions that will provide a provision that is holistic, evidenced based and delivered within communities.
