Full Job Description

Please be aware this is a Long Term Temporary post to start ASAP.

- SWE Registration

- Right to work within the UK

Purpose of the Post:

To undertake needs-led assessments, duty screening and assessment visits to provide support to adults and their carers in both crisis and pre-planned situations.

To provide specialist support to service users, carers and colleagues in accordance with their particular skills and qualifications.

To work in an Integrated Team to provide the required support in delivering the NHS Framework for Continuing Healthcare and NHS Funded Nursing Care for Adults.

To work within a multi-disciplinary setting; actively advocating the on-going development and implementation of integrated and partnership working.

Duties and Responsibilities:

Complete complex multi-disciplinary assessments (including risk) and provide on-going case management for people who may have unstable and unpredictable conditions, within the criteria of each individual service.

To offer and conduct assessments for Carers in relation to their own needs as distinct from those of the individuals being cared for.

Where possible seek to sustain and maintain individuals in the community who's challenging behaviours present a risk to the individual and others.

To identify eligible needs and / or outcomes, and identity appropriate services to meet those needs / outcomes including Self Directed Support options.

To work with the Adult Safeguarding team during investigations where appropriate.

To act as Best Interest Assessor or be prepared to undertake training in order to act as a Best Interest Assessor and make recommendations in line with Deprivation of Liberty Safeguards.

To make decisions in relation to the Mental Capacity Act regarding best interests and capacity, without support.

Provide informal supervision in the office environment.

Supervise student social workers.

Deputise for the Team Manger or Assistant Team Manager when appropriate.

Work in-line with departmental policy and procedure, within the social care legislative framework and within set budgets.

To act as a service broker where appropriate to arrange, commission and co-ordinate appropriate services according to:

Any agreement made by the Authority with service providers

Agreed budget constraints

Principles of Self Directed Support

To participate in staff appraisal, supervision and training including PQ as appropriate.

To participate in developmental activities as may be required to promote improvement of service or the use of resources.

To accurately record, critically analyse and keep up to date information using the appropriate IT systems in accordance with the Departments recording policy. To produce relevant reports, including court statements to a high standard, as required.

To comply with all the council's financial regulations.

To fully participate in supervisory processes and training opportunities for the purpose of improving practice and consolidating professional development.

Plan, review and organise workload to ensure deadlines are met and appropriate actions taken in a timely manner and manage conflicting priorities within your caseload, in conjunction with line manager.

Develop own abilities and decision making in conjunction with Team Manager.

Fully participate in the day to day responsibilities of the team commensurate with your role.

    Adults, BA in Social Work or equivalent, Children, Duty and Assessment, Learning Disabilities, Mental Health, Qualified Social Worker, Skills, A professional qualification in social work

  • Social Work England registration.

  • Post qualified experience.

  • Enhanced DBS/Update service.

  • References required for the last 5 years of employment.
