Full Job Description
Deliver person-centred and strength-based interventions under the Care Act (2014).
Carry out Care Act (2014) assessments and complex reviews.
Build strong, collaborative links with carers and partner agencies to maximise independence and individual wellbeing.
Identify signposting opportunities to ensure carers and individuals receive appropriate support.
Cover areas including Brampton and Walton, Barlow and Holmesfield, Coal Aston, Gosforth Valley, Ridgeway and Marsh Lane, Unstone, Dronfield, Killamarsh, Eckington, Renishaw, and the borders of Sheffield.
Travel across the designated areas to provide support and interventions as required.
Manage a complex caseload while meeting statutory duties in a safe, timely, accountable, and cost-effective manner to meet eligible needs.
Take the lead on Section 42 Safeguarding enquiries and investigations.
Chair multi-agency meetings, such as Vulnerable Adult Risk Management (VARM).
Complete Mental Capacity Act (2005) assessments, including Best Interest meetings, COP-DOLs, and court work.
Respond to changing situations, including Duty cover to effectively triage incoming work and support Adults and their carers who may require a more urgent response.
You are an experienced Social Worker with a proven ability to manage cases independently and effectively.
You have excellent communication and relationship-building skills.
You can demonstrate a commitment to promoting individual wellbeing and achieving strength-based outcomes.
You have the ability to travel across the areas covered by the team.
You thrive in a collaborative environment and value working closely with a wide range of partner agencies.
You have knowledge and experience in safeguarding, multi-agency collaboration, and the application of the Mental Capacity Act (2005).
