Are you interested in...

  • Working with allocated individuals who have social care needs and serious and enduring mental health difficulties?
  • Developing and providing social care/health evidence based interventions for individuals of working age with mental health problems who are receiving social care/health funding and managed through the Social Care Hub?, Manage the service provision for a defined caseload of individuals who have social care/health needs as specified in the Social Care Hub Operational Policy.
  • To act as Care Manager where appropriate.
  • To ensure all child protection, adult safeguarding and Mental Capacity Act issues are dealt with according to policy.
  • To respond to requests for assessments and review of need for people with social care needs. Working in partnership with service users, carers and other professionals as agreed within the multi-disciplinary team and with the Recovery and Assertive Outreach Team Managers, including out of county/area reviews.
  • To work in partnership with service users, carers, other team members and other statutory and non-statutory agencies.
  • To liaise closely with Primary Health Services and other agencies at the point of discharge to ensure a smooth transition.
  • To participate in caseload supervision and workload management on a regular basis as required by the Social Care Hub Team manager.
  • To positively address issues of discrimination and social exclusion.
  • To act as mentor, supervisor and assessor to social work students and ASYE socialworkers who are placed within the team.
  • To participate in supervision, both personally and as a supervisor for junior staff within the requirements of Trust policy
  • To act up for the Social Care Hub Manager as required
