Full Job Description

We are currently recruiting for a Social Worker within our integrated discharge team to contribute to the delivery of highly effective children and adult services (within Adult Social Care) in an efficient manner. The service is delivered using a strength based, personalised approach. We work across both the acute and non-acute sites across Medway and out of area hospitals. We have been at the forefront of changes in the hospitals discharge to assess model. This has meant that our teamwork with people in an acute setting as well as those in community settings after a period of the time in hospital. This can either be in a health bed, social care step down bed or in the persons' own home. This role also includes a role in the emergency department of the acute hospital by working with our health colleagues. To work closely with colleagues within the Council and partners ensuring that person centred outcomes are met in ways which utilises available resources as effectively as possible. To work within organisational policy and legislative requirements including The Care Act, Mental Capacity Act and Mental health Act. Medway is a vibrant and exciting place to work, and an opportunity has arisen to join the hospital discharge to assess social work team. We are a supportive team working in a busy environment whilst maintaining our professional standards and we are committed to ongoing professional standards. The successful candidates will be required to work across all settings in line with the changing demands of the system to enable the discharge to assess model to be implemented to ensure timely and safe discharges. All staff working in the department have a responsibility for promoting and supporting the Council's policies and procedures for safeguarding. You should ensure that you carry out your duties and work at all times in a way that ensures the safeguarding and welfare of service users.
