Full Job Description
The role is for 37 hour per week on a six-week rota which includes working 8am to 4pm or 12pm to 8pm Monday to Friday and 8am to 4pm Saturday and Sunday. Social workers should be available to work a variety of the shifts across the rota. Please specify your desired working hours in your personal summary if you would like us to consider an alternative working pattern. You must work at least two bank holidays and eight weekends per annum to be eligible for the market supplement. - T&C's Main Responsibility
- Carry out strengths-based and person-centred assessments with individuals and their carers to support discharge planning as outlined under the Care Act 2014 and Health and Social Care Act 2022 with individuals and their carers.
- Attend board rounds, multi-disciplinary meetings and discharge planning meetings to support the day-to-day operational duties of the Integrated Discharge Hub.
- Work in partnership with health and social care colleagues within the Integrated Discharge Hub to collaboratively ensure safe and timely discharges for people.
- Complete safeguarding enquiries, mental capacity assessments and best interest's decisions with guidance from line manager.
- Manage complex case load and the competing demands and time pressures of arranging timely discharges with guidance from line manager.
- Attend and contribute to regular supervision meetings to ensure effective communication and informed decision making.
- Be responsible for updating own development, sharing best practice and case law to maintain high standards of care.
Detailed knowledge of legal frameworks and policies such as the Care Act 2014, Mental Capacity Act 2005, Health and Social Care Act 2022 and Hospital Discharge Guidance. - Experience in hospital discharge and working as part of a multidisciplinary team is desirable.
- Qualified Social Worker registered with Social Work England.
- For this role, the candidate is required to have a full UK driving licence, along with access to a vehicle. Evidence of the driving licence and vehicle insurance with business insurance will be required from the candidate should they be offered this position. Without this documentation the candidate will not be appointable.
