Full Job Description

An opportunity has arisen to join the acute frailty pharmacy team as one of the Specialist Clinical Pharmacists - Acute Frailty for 9 months covering the absence of the permanent staff member. Along with the other two Specialist Clinical Pharmacists - Acute Frailty and the Chief Technician - Acute Frailty you will lead the pharmacy service provision to Acute Frailty Pathway at NUH. The job will be based on the newly opened Geriatric Assessment at Queens Medical Centre where you will work as part of the MDT providing high quality medicines reconciliation and pharmaceutical knowledge. The post holder will work with the other members of the MDT to perform medicines optimisation, including deprescribing and prevention of harm from inappropriate polypharmacy. The post holder will also work within the wider pharmacy Healthcare of the Elderly (HCOP) and medicine Integrated Care pharmacy team, receiving and giving support to the other team members. This position offers the opportunity to make a real difference to the care of Frail patients admitted to NUH., For full duties please refer to the attached job description. Key duties are:

  • Provision of a clinical pharmacy service to the acute frailty pathway at Nottingham University Hospitals, including medicines reconciliation, medicines optimisation and safe transfer of care.
  • Effective team working within the NUH Frailty team and wider pharmacy team to ensure good patient care
  • Support and provide education and training to other staff members
  • Provide specialist pharmacist input into the governance, guidelines and service development of the NUH Frailty service
  • Participation in the NUH pharmacy 7 day working rota and on call back up provision.
