Full Job Description
The post holder with work closely with the current Advanced Specialist Pharmacist - Perioperative Medicine (8a) secondment, Advanced Specialist Pharmacist, Trauma and Orthopaedics, and the whole pre-assessment nursing/anaesthetic team to develop medicines optimisation service to support the elective recovery workstream. This will include review of specific conditions to support with streamlining the current process for review, optimisation, and escalation, including hypertension, COPD, diabetes and heart failure., The post will primarily be based at County hospital, supporting the elective orthopaedic unit (EOU), although there is scope to provide cross-site cover. The post-holder will be responsible for regular attendance on the EOU daily ward round and mentoring the band 6 rotational pharmacist routinely covering the EOU ward.
To effectively deliver a pharmacy pre-AMS service, you must have obtained an independent prescriber qualification or will be supported by starting and completing an independent prescribing course. Being an active prescriber is a key activity and we will expand and utilise these skills in this role. This role provides a unique opportunity to be able to directly shape new and existing pre-AMS services.
The focus of the pharmacy Pre-AMS will be to ensure safe and effective prescribing and safe and timely administration of medicines during the peri-operative period. This will include establishing pre-admission medicines history, documenting this and ensuring the patient has been given the correct advice relating to their medicines in the period up to their admission and procedure.
