Full Job Description

The post holder will be the lead Commissioner in a very busy and varied role overseeing Trafford's older persons' market. The main areas of responsibility will be Nursing, Residential Care, Extra Care, Discharge to Assess and Home Care. The postholder will be key to driving forward an agenda which supports an improved and sustainable bed based and community offer for our residents, with efficient and effective contract and financial management, quality assurance and innovation.

Working closely with our Adult Health and Social Care colleagues, you will work on our key priorities developing our commissioning approach so that we make a difference for people needing our services, ensuring we have a sustainable offer to support the wider system across Trafford. There will be a focus will be Hospital Discharge processes and admission avoidance, Quality Assurance, and Discharge to Assess, Extra Care and transforming homecare and reablement provisions together with improving our community offer for our residents; ensuring that our local providers provide a comprehensive high quality person-centered range of support.

You will support our ASC colleagues with complex cases in respect of discharges, ensuring safe and efficient discharges, positively challenging provider decisions where necessary to expedite those discharge arrangements, particularly at periods of system escalation. You will oversee the continued development of the Quality Assurance Framework for Commissioned providers, together with our Provider Concerns system to collate intelligence to drive quality improvement.

You will oversee the commissioning process, including managing service reviews, developments and innovations to maximise outcomes, ensure value for money and service improvement.

You will support your commissioning managers to achieve continuous service improvement through consistent contract, quality and performance management of a portfolio of providers or services.

About the Role

As a specialist commissioner you will lead a portfolio of commissioning activities, related to older people in Trafford and have extensive experience and knowledge and a significant understanding of the key issues relevant to commissioning.

You will ensure all commissioned services are both delivering value for money and high-quality services which deliver agreed outcomes. This will include oversight of key aspects of the Trafford 2031 vision, the health and social care system and substantial commissioning budgets.

You will also work closely with key partners within Trafford, particularly leading on strategic initiatives relevant to the integration of health and social care in Greater Manchester, working closely with partners across the North West. This will include managing resources (staff and budgets) as well as partnership contributions.

You will be able to demonstrate being able to plan and prioritise and work to tight deadlines. Working with the other Specialist Commissioners in the commissioning service, you will ensure appropriate integration with the other thematic areas within the services for adult social care, children's health, children's social care and public health portfolios.

With particular reference to Residential, Nursing, Homecare, Extra Care, Reablement and Discharge to Assess you will review our current offer in line with the Trafford Locality Plan which summarises our ambition to support people to live at home for longer by ensuring more robust locality-based services which will reduce the need for more restrictive services, reduce hospital admissions and provide a step-down offer of support.

You must be able to demonstrate experience in managing and developing services, alongside leadership abilities and excellent communication and interpersonal skills.

You should also have a minimum of 2 years' management experience, ideally within the residential, nursing, homecare or commissioning sector and hold a minimum of a degree or other recognised professional qualification. Project management experience is required for this post., You will be committed to making a difference through your work. You will be able to demonstrate a track record of working in partnership with providers and residents to deliver change which improves outcomes for residents.

You will have extensive experience and knowledge in the varied aspects of commissioning.

you will be able to focus on your area of responsibility with the ability to think and plan strategically towards improvements and efficiencies.

You will take personal responsibility to resolve issues and change what you do and how you do it, to be effective.

You will have a good grasp of data and finances and be able to develop costed proposals as part of your work together with sound negotiation skills.
