Full Job Description
Chippenham Community Team has an exciting opportunity for a part time Band 6 Specialist Community Occupational Therapist to join our team. If you are passionate about delivering community-based therapy, improving quality of life for individuals, and are committed to rehabilitation in a home environment, we would love to hear from you. The post is for 16.5 hours per week.
Our multi-disciplinary team is based in Chippenham Community Hospital and provides complex interventions to a wide range of patients in their own homes and other community clinical settings. Our therapy team consists of 6 therapy staff who manage patients from 5 local GP practices. Our therapy service is provided between the hours of 08:00 and 18:00 including some weekends but this is flexible. We also work closely with our wider partners including Specialist Services, Social Care, Acute Hospitals and Primary Care.
You will be working within a friendly and supportive team andthe role includes a focus on rehabilitation, long term condition management and supporting hospital discharges. You will be required to hold a complex clinical caseload and provide occupational therapy assessments to identify goals, formulate and deliver individual treatment programmes and order a wide range of equipment., The role also includes responding to urgent referrals, liaising with a wide range of colleagues to plan supported discharges, supporting other staff members and being involved in ongoing processes to provide a continual proactive and effective service.
This is an ideal opportunity for an experienced Occupational Therapist keen to develop enhanced skills and knowledge as part of a dynamic, multidisciplinary team. You will need to have good organisational and IT skills combined with experience of working with adult and older persons services. Excellent clinical and communication skills and the ability to work under pressure are also required. We are looking for someone who is enthusiastic and who can work flexibly and is willing to contribute to the ongoing development of the services we provide.
