Full Job Description

Are you a qualified (or soon to qualify) Occupational Therapist looking for a varied and rewarding community role with lots of learning opportunities? Are you compassionate, enjoy problem-solving and motivated by enabling individuals to thrive? Join our collaborative and welcoming Community Team for People with Learning Disabilities (CTPLD). We're based in St Marks Hospital, Maidenhead and work with adults in the Windsor & Maidenhead area. In this role you'll:

  • Use a holistic approach to deliver creative, person-centred and evidence-based interventions that are tailored to individuals with learning disabilities, including those with complex needs
  • Be autonomous and manage your own diary, working across community settings such as homes, supported living services, and day centres
  • Advocate for and enable people to achieve their potential, access opportunities and maintain good health
  • Work with your local supportive multi-disciplinary team on our referrals, have professional support from your OT peers and senior colleagues in the wider team and benefit from our Trust's exceptional AHP structure
  • This is a great team for a Band 5 OT to develop wide ranging OT skills within a supportive culture. You'll also have opportunity to cross cover in other teams in the Trust. Our usual service hours are Mon - Fri 9-5 with options for home and flexible working. Applications are processed as they are received.   • Work autonomously within a team, under the support of a Band 6 OT, to contribute to the development of the Specialist Occupational Therapy Service in WAM CTPLD.
  • Collaborate with clients, families, carers, and other professionals to assess, plan, implement, and review Occupational Therapy treatment plans for individuals with learning disabilities.
  • Manage a caseload of clients with complex needs, using person-centred, evidence-based approaches to create and evaluate individualized interventions.
  • Provide specialist clinical advice and training to colleagues, carers, and professionals, including students.
  • Set specific, relevant goals and conduct care assessments at the start, during, and upon discharge for each referral.
  • Maintain accurate client records and update Continuing Professional Development (CPD) records.
  • Work collaboratively within a multidisciplinary team, ensuring a holistic approach to client care
