Full Job Description

We are seeking someone who is passionate, creative and has a drive to work closely with families and education provisions to support children, young people and their families to be as healthy as they can be. Collectively, these skills and approach to your role will allow you to really make a difference to the life chances and outcomes for the young people in Doncaster, supporting the vision of making Doncaster the best place to live.
The post holder will work across all schools, colleges and alternative education settings within Doncaster, managing a universal caseload of children and young people, as well as a focused caseload of vulnerable and complex children and families. The post holder will be the named SCPHN for an identified pyramid of schools and will also work along side the Team Lead to support the day to day work of a wide skill mix of colleagues and students, using co-ordination and supervision skills
In addition, the post requires an in-depth knowledge of The Healthy Child Programme, Public Health data and drivers as well as safeguarding practice., The post holder will provide an holistic service for children, young people and families who live in Doncaster and attend a Doncaster school, other alternative education setting or who are educated at home.
To lead on the assessment of health needs and support in the planning, implementation and evaluation of individualised packages of care.
To lead and facilitate each school to have an individual School Health Profile to identify the health needs of the school population and geographical area.
Provide training and support for children, young people, parents/carers, and school staff to meet individual health needs.
To work collaboratively with partner agencies to provide an innovative approach to delivering an enhanced public health service to the children and young people who attend schools and other educational settings as well as those educated at home.
