Central London Community Healthcare NHS Trust (CLCH) is responsible for delivering a patient-focused rehabilitation service for West Hertfordshire. St Peter's Ward provides a dedicated community-based step-down facility, enabling patients to continue their rehabilitation and recovery in a supportive community setting with access to a structured therapy program. This role involves conducting ongoing assessments for patients who are medically stable but require further evaluation of their health, social needs, and potential future living arrangements, including Continuing Healthcare (CHC) assessments, outside of the acute hospital environment. St Peter's Ward operates 24/7, ensuring round-the-clock nursing care with access to doctors and therapists during core hours to maintain a high standard of patient support.Main duties of the job Manage rehabilitation patients' medical care as the sole medic resident, with Consultant support twice weekly. Address medical deteriorations under Consultant guidance to optimize rehabilitation, considering frailty and comorbidities; treatment options vary by severity. Prevent unnecessary hospital admissions through daily patient reviews. Ensure optimal patient discharge locations, including home. Support patients, families, and carers. Create a flexible, recovery-focused environment responsive to changing needs. Conduct patient admissions, including medical histories, examinations, SystmOne note entry, and prescribing. Perform daily ward reviews and attend ward sweeps. Assess manageable minor issues in the community. Maintain timely notes in SystmOne. Order and review non-urgent tests like blood tests and imaging. Participate in weekly MDT meetings. Update families on medical care as needed. Assess unwell patients referred by nursing/therapy staff. Advise nursing/therapy staff on medical management. Collaborate with the ward pharmacist on medication management issues. Refer patients to specialists or emergency departments as necessary. Communicate with hospital teams and prepare discharge summaries and TTAs promptly.
