Full Job Description
We are currently looking to recruit a motivated and experienced Band 6 Mental Health (MH) Practitioner to join our Wisbech Neighbourhood Team (NT) Older People's Service. This is a great opportunity to join our effective and friendly team working with a client group with more complex mental health needs. You will not only be part of an integrated team of mental Health practitioners including CPNs, psychologists, Consultant Psychiatrist, Support Worker, Social Workers, but will be collocated with district nurses and therapy staff, enabling effective coordinated care delivery. We provide service-user focused, evidence-based interventions for people aged 65 and over living in their own homes/care homes with either functional or organic mental health illness. We provide high quality assessment, treatment, and care coordination which is informed by your professional bodies and National policy. The post has opportunities to develop skills and knowledge via training, working alongside experienced practitioners and to be involved in service improvement. You will have the opportunity to manage a caseload of service users, delivering a diverse range of clinical skills delivering therapeutic and recovery-based interventions. We seek to empower service users and carers to actively engage in their recovery journey, working alongside a range of third sector organisations. The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by an ability to work under pressure at times, and with a desire for innovation. We support agile working for staff, using all appropriate I.T equipment and support. This service covers Wisbech and the surrounding rural areas. - Mental Health practitioners in Neighbourhood Teams will provide assessment and interventions for people with significant mental health conditions
- Offer advice and guidance to colleagues in the neighbourhood team on common mental health conditions.
- Participate in MDT meetings and ensure key information is recorded in shared care plans
- Work closely with the mental health integrated care team in order to provide an excellent mental health service.
