Full Job Description

We have an exciting opportunity for a Physiotherapist to join our award winning Community Neurology team, based in King's Lynn.

Our multi-professional team provides a specialist service for individuals across West Norfolk with a variety of neurological conditions for example MS, PD, MND and TBI.

The role includes working across the whole patient pathway from diagnosis and new symptoms management, through the condition pathway home based therapies, case management, social re-integration, vocational rehabilitation and palliative care. The post holder will be supported to provide specialist neurological assessment, interpretation, intervention and education. This will include patient-centred goal planning and rehabilitation and liaison/referral to other support services, and family support where appropriate in conjunction with the rest of the team.

Assessment and intervention will predominantly be held in patients home or at patients' choice of venue, including the gym, and other sites in the community where appropriate.

Main duties of the job

Undertake a major clinical role in assessment, review and treatment to the adult population with the diagnosis of a Neurological Condition and problems related to this, living in the community within West Norfolk.

o To plan, deliver and co-ordinate Physiotherapy intervention for patients with Neurological Conditions across West Norfolk

o To be responsible for Continued Professional Development (CPD) and Personal Development Plan (PDP) for Community Assistant practitioners.

o To have an input into service development, under the direction of the Team Leads/ Service Manager, across West Norfolk and in conjunction with the rest of the team in response to NICE guidance, local governance and government policy.
