Full Job Description

This is an exciting opportunity for an experienced nurse to join our dedicated Looked after Children's nursing team within Kensington and Chelsea, Westminster and Hammersmith and Fulham. We are an integrated team working with a range of providers to include professionals from education, social care and health.
The post holder will be responsible for a caseload of looked after children and young people. It is expected that the post holder will autonomously undertake direct specialist clinical work including carrying out statutory health assessment reviews on Looked after Children. You will also be delivering training to foster carers and other health and social care professionals. An interest and knowledge of looked after children and young people are essential.
Experience of safeguarding and multi-agency working is also key to the role. The team also take responsibility for children placed out of borough. This includes undertaking the health assessment and/or liaison with other agencies and services, coordinating effective care and acting as an advocate where necessary.
The post holder will provide expertise and demonstrate sophisticated clinical skills to work directly with vulnerable children and adolescents from a background of trauma and/or neglect.
You will be joining an award winning team who are dedicated to providing excellent health care to Looked after Children., To work actively in conjunction with colleagues providing health care advice and information and work towards achieving You're Welcome Standards for making services young people friendly.
To promote health through formal and informal health education with relevant individuals and groups, promoting an understanding of the health needs of Children in Care.
To carry out health assessments and review health care plans of Children in Care as appropriate; to contribute and improve the quality and uptake of rate of health assessments and immunisations
To provide direct specialist clinical input to highly complex Children in Care cases and consult, liaise and assist other colleagues in their clinical work with Children in Care in the community.
To assist with training and updating of the health issues for Children in Care for GP's, Health Workers, Social Services, Education and other partner agencies.
To form and maintain collaborative working relationships with Social Services, Education in particular and other relevant agencies, aiming to improve access to healthcare.
To promote partnership with relevant health services for improving health outcomes and life chances of Children in Care. E.g., Child and Adolescent Mental Health Services, Education, Teenage Pregnancy Strategy, Sexual Health, Dietetics, Dental Health, Drugs and Alcohol Service., Interview arrangements will be communicated via email so please check your email regularly following the closing date.
