Full Job Description

To be hospital based for the majority of the time by providing direct specialist care, education and support for patients with transitional care and acting as a specialist resource when liaising with and facilitating the best use of all available services

- To provide care for transitional care patients, in accordance with national and Trust policies and Frameworks.

- To undertake a needs-based assessment of the current facilities, services available to patients and their families during transition and to continue to develop the service to assist patients during their transition phase

- Where possible to support young people 16 years and above when placed onto an adult ward within the Trust;

- Provide independent and unsupervised support to patients, families when preparing the patient and their care group through the transition period

- To be flexible in the approach to the clinical area and provide clinical support as required

- In conjunction with the ward managers and specialist nurses/matron provide a suitable and safe environment for the physical and psychological wellbeing of staff, patients, and their relatives/carers.

- Be responsible for keeping up to date with the transition agenda with NHSE.

- Act as an expert, clinical role model and leader in transitional care for all staff both locally and within the wider NHS community

We are seeking an enthusiastic individual to Lead the Transition service for children with complex needs moving into adult services.

The applicant will have previous hospital nursing experience, working with children and young people, experience working in a transitional role for children moving into adult services.
