Specialist Nurse / Paramedic EIV

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
We are seeking enthusiastic senior nurses/paramedics to join our Early Intervention Vehicle (EIV) Service, delivering care through the Unplanned Care Team.
The purpose of the EIV is pathway is to deliver crisis management to patients living in Herts Valleys & prevent hospital admission, providing clinical assessment by a nurse/paramedic jointly with staff from Herts County Council.
The EIV pathway is closely aligned to other Unplanned Care services, which deliver high quality clinical care to patients in their own home at times of crisis. The purpose of the Rapid Response (RR) pathway is to prevent hospital attendance/admission and the Discharge to Assess (DTA) pathway is to facilitate early discharge from hospital. The EIV will supplement and support the delivery of Rapid Response and Discharge to Assess in the Unplanned Care Service, often referring patients onwards after assessment.
The EIV delivers care to patients across all four Herts Valleys localities, responding to non-life-threatening priority calls. The practitioner, will carry out a holistic physical & functional assessment, providing any necessary urgent equipment to ensure they remain at home & identify further needs- making the necessary onwards referrals., The post holder will work autonomously and in collaboration with other health care professionals, statutory and voluntary agencies and the RR team to deliver excellent standards of patient care for patients requiring support in a crisis.
To respond rapidly and deliver high quality, skilled clinical assessments to patients at their usual place of residence, providing emergency equipment and assistance where required.
The service operates through pre-rostered shift patterns spanning 8am - 8pm 7 days a week (subject to extend once fully established).
To utilise specialist skills and knowledge to holistically assess needs.
To work, as required, on a flexible basis to cover all roles required within the team including RR team if required.

The successful candidate will be a registered nurse or paramedic with advanced clinical skills, significant knowledge and experience in the clinical care of adults and older people in a range of healthcare settings and the management of long term conditions.
You will be self-motivated with strong organisational skills and able to make complex clinical decisions quickly and autonomously as required. As a senior clinician you will use your advanced clinical expertise, levels of judgement, discretion and decision making to support patients at home, improve patient outcomes and reduce likelihood of unnecessary admissions to hospital. You will be required to complete holistic assessments, giving appropriate interventions as required and have knowledge of community services to plan the on-going care of patients on the pathway.
This is an exciting post, with the opportunity to help develop the service and make a real difference to patients and a chance to develop both clinical and strategic skills.
