Full Job Description

An exciting opportunity has arisen for a motivated and enthusiastic registered nurse to join the Rheumatology Team, working across site as a Specialist Nurse in Osteoporosis and DMARD monitoring. alongside the Rheumatology Consultants and other Specialist Nurses. The role will encompass osteoporosis and the monitoring of Disease modifying drugs. This will enable us to provide high quality care following national and local guidelines. You will be working cross site with Rheumatologists and Rheumatology Specialist Nurses, to undertake the arrangement of various osteoporosis treatments acting as a source of expertise and knowledge through the provision of education and support.

You will be monitoring patients who are initiated on DMARD's by the Medical team providing a liaison between outpatient's clinics, day unit therapy, inpatient and primary care ensuring continuity of care for patients undergoing osteoporosis treatment and initiating DMARD's., Working across site with the Rheumatologists and Rheumatology Specialist Nurses, to undertake the arrangement of various osteoporosis treatments acting as a source of expertise and knowledge through the provision of education and support.

Monitoring patients who are initiated on DMARD's by the Medical team

Providing a liaison between outpatient's clinics, day unit therapy, inpatient and primary care ensuring continuity of care for patients undergoing osteoporosis treatment and initiating DMARD's.

The post holder will work across site as necessary between Eastbourne District General Hospital/Conquest encompassing Uckfield Hospital as the service requires and develops.

You will be expected to have excellent organisational, communication and interpersonal skills and be able to work both independently and within a team. Ideally you will have education to degree level and evidence of continuing professional practice. Experience of the complexities of rheumatological conditions would be an advantage.
