Full Job Description

We are seeking an enthusiastic occupational therapist to join our Planned Care team for a fixed term.

The position offers experience of a mixed caseload of patients with various pathologies; including amputees, neurological / stroke rehabilitation, Parkinson's disease, falls / balance impairments, orthopaedics, long-term conditions and palliative / end of life care. The aim is to rehabilitate and maximise the patients' potential, to support and enable patients in a range of activities to promote independent living in their home environment.

The team is a well-established and covers a large area and population size, the position requires the delivery of assessment and treatment of occupational therapy, providing equipment for patients in their own homes, liaising with patients, family, carers and other MDT staff.

This is an excellent opportunity for an OT working in the acute, bed-based rehab, or social services who would like to experience community working, without the long term contract.

A full valid driving license and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

Main duties of the job

Provide assessment, treatment, and interventions to patients in their own home environment to support them to optimise their functional ability.

You are required to be a car driver and have access to a car to use for work to see patients in their own home across the locality. You may be required to transport and provide equipment from our satellite stores to the patient.

You will work as an autonomous practitioner, but you will work closely with other OT's, assistants and physio's.

The team also has close links with the community nurses, complex care and frailty team, GP's, palliative care specialists, hospices, social care, and other third party sector organisations.
