Full Job Description

The Community Stroke Rehabilitation are looking for a qualified Occupational Therapist with experience in stroke rehabilitation to join a successful team.

The post is based at either of our two bases: Chester le Street Hospital or Sedgefield Community Hospital, treating patients in and around the Durham Dales, and North Durham area. The successful candidate will work alongside Physiotherapists, Speech and Language Therapists and skilled associates to deliver innovative, evidence-based stroke rehabilitation in patients own homes and out-patient settings to optimise recovery.

This is a full time permanent post Monday to Friday 0830-1630.

The role is diverse and challenging and we are looking for someone to join our friendly team and to meet the needs of patients and bring ideas to the service regarding service delivery and development. This is an exciting opportunity for a candidate to apply their skills in a flexible, innovative manner and to create solutions to offer our patients an optimal rehabilitation journey., The succesful candidate will be expected to deliver high quality rehabilitation to stroke survivors throughout North Durham as part of a multi- displinary team:

-Assessing and reviewing complex stroke survivors

-Manage own diary and caseload of patients and contributing to the MDT

-Own individual responsibilities and share team objectives

-Lead or contribute to creating improvement, transformation and innovation of services

-Contribute to creating a work environment that is marked by pride, enthusiasm and collaboration

-Manage and/or contribute to financial performance and target delivery

-Lead by action and inspire others acting as role model, demonstrating the leadership behaviours and values identified by CDDFT

-Communicate positively and effectively

-Actively give and receive feedback in a constructive manner

-Be adaptable, work with integrity and be trustworthy

-Show constancy, courage and resolve in the pursuit of the vision and aims of CDDFT

-Actively contribute to improving the health and well being of the population that is served

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
