Full Job Description

Neighbourhood Working describes an integrated approach to managing patients, through a blended workforce that encompasses both health and social care; to include acute, voluntary and community sectors where barriers to working are negated, the health and wellbeing needs of the individual are at the centre of decision-making, care is proactive and not reactive, and services are provided in a timely manner. This is a community-based post providing initial & on-going assessment and support to people referred through Primary Care and Neighbourhood Team practitioners. The post will sit within the Grantham & Sleaford Neighbourhood Team, supporting local populations in managing challenges experienced from physical and mental health conditions.,

  • To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive personalised care and support planning using highly developed clinical reasoning skills and appropriate assessment tools.
  • This role will work with adults from 18 years upwards where a reduction in occupational function and performance is identified resulting from frailty syndromes. Some flexibility to work flexible hours will be required.
  • To delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility for patients care, where appropriate.
  • To ensure the clinical caseload and clinical practice of occupational therapists in the team is of the highest standard of clinical care.
  • To manage referrals and service delivery in accordance with and in order to contribute towards the attainment of local organisational productivity requirements.
  • To communicate and liaise with others to ensure appropriate assessments, interventions and reviews are in place.
  • Work with individuals to assess their mental and physical health, recognise mental and physical illness and identify their related goals. Plan, implement, review and improve interventions to meet peoples identified needs and manage any associated risk.
  • To assess carers and family's needs and signpost or refer them to the relevant support.
  • To identify and act on safeguarding concerns using current pathways.
