The salary scale above has been agreed as part of the NHS Agenda for Change pay award for 2024/2025 and will be implemented in November 2024 backdated to 1st April 2024 where applicable., This is an opportunity to join our community and primary care occupational therapy service in the East IHC. The post will involve working closely with GP and district nursing colleagues in primary care to continue to build on good MDT working., 1. To provide holistic OT assessment, goal setting, individualised treatment and rehabilitation programmes, care planning and delivery.
2. Regular review and discharge of patients.
3. To manage a varied caseload, seeking support from senior staff where necessary to ensure the provision of a timely, clinically effective service based on evidence based practice.
4. To be actively involved in, and lead on, projects towards the development & delivery of the service and to contribute to new developments and initiatives.
